How to submit a request

Download the form

Download the official ClassCover Data Subject Request Form below. It's an Excel file you can fill in on your computer.

Fill in your details and request type

Complete Parts 1โ€“5 of the form โ€” your contact details, what you're requesting, a brief description, identity verification, and your declaration.

Email the completed form to us

Send the filled-in form to info@classcover.com.au with the subject line "Data Subject Request". Please include a copy of a valid photo ID so we can verify your identity.

We respond within 30 days

Under GDPR, we are required to respond to your request within one calendar month. We will confirm receipt of your request and keep you informed of progress.

Identity verification: To protect your data, we must verify your identity before processing any request. Please attach a copy of a valid photo ID (e.g. passport or driving licence) when emailing your form. We will not process requests where we cannot confirm the identity of the requester.

Right to complain: If you are unhappy with how we handle your request, or believe your data is being processed unlawfully, you have the right to lodge a complaint with a supervisory authority. In Ireland, this is the Data Protection Commission (DPC).
